Comparative Efficacy of Sacubitril/Valsartan Sodium versus Perindopril in Elderly Patients with HFrEF and Hypertension

Abstract

Objective: To compare the efficacy of sacubitril/valsartan sodium versus perindopril in treating elderly patients with heart failure with reduced ejection fraction (HFrEF) and hypertension. Methods: A total of 100 patients with HFrEF and hypertension admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University between January 2024 and June 2025 were enrolled. Using the random number table and sealed-envelope allocation concealment method, they were divided into two groups: group A (oral perindopril, n=50) and group B (oral sacubitril/valsartan sodium, n=50). The therapeutic efficacy, cardiac function, blood pressure, serum biomarkers, exercise endurance, quality of life, and prognosis of the patients were compared between the two groups before and after treatment. Results: Compared with group B, group A had a lower total effective rate, and a higher total incidence of adverse reactions and proportion of heart failure readmission within 6 months, with statistically significant differences (P<0.05). Group B showed superior improvements in cardiac function (higher LVEF and lower LVEDD), blood pressure control (lower systolic and diastolic blood pressure), exercise endurance (longer 6MWD), inflammatory and fibrotic biomarkers (lower levels of NT-pro BNP, Gal-3, and IL-6), and quality of life (lower MLHFQ score) compared to group A (P<0.05). Conclusion: In elderly patients with HFrEF and hypertension, sacubitril/valsartan sodium demonstrates better efficacy, safety, and short-term prognosis, and may be considered as a preferable treatment option for this population.
