Treatment of Myocardial Damage with Fructose 1,6-diphosphate following Neonatal Asphyxia: A Systematic Review and Meta-analysis

摘要: 目的:评价1,6-二磷酸果糖(FDP)治疗新生儿窒息后心肌损伤的疗效。 方法:检索PubMed、EMBASE、Cochrane Library、中国生物医学文献数据库、万方数据库和中国知网中FDP治疗新生儿窒息后心肌损伤的所有随机对照研究,检索时间从建库至2017年4月。按照纳入及排除标准独立筛选文献,同时提取纳入文献的基本资料、评价偏倚风险,采用RevMan 5.3软件进行Meta分析。 结果:本文Meta分析最终纳入24篇研究文献(n=1754)。Meta分析结果显示:(1)治疗疗效:FDP治疗新生儿窒息后心肌损伤的显效率[OR=3.31(95%CI:2.71~4.04),P<0.00001]差异有统计学意义;总有效率[OR=5.59(95%CI:4.13~7.57),P<0.00001]差异有统计学意义。(2)不良反应:纳入文献中3篇报道均无不良反应,其余文献未见报道。 结论: FDP可显著提高新生儿窒息后心肌损伤的显效率及总有效率。

Abstract: Objective: To evaluate the efficacy of fructose 1,6-diphosphate (FDP) in the treatment of neonatal asphyxia. Methods: All randomized controlled trials of PubMed, EMBASE, Cochrane Library, Chinese biomedical literature database, Wanfang database and FDP in the treatment of neonatal asphyxia were investigated until April 2017. The literatures were independently screened according to the inclusion and exclusion criteria, and the basic information of the literature was extracted. The risk of bias was evaluated and Meta- analysis was performed using RevMan 5.3 software. Results: Meta-analysis of this article eventually included 24 studies (n=1754). Meta-analysis showed that: (1) Therapeutic effect: the efficacy of FDP in the treatment of neonatal asphyxia (OR=3.31 (95% CI: 2.71~4.04), P<0.00001] was statistically significant and the total effective rate [OR=5.59 (95% CI: 4.13~7.57), P<0.00001] were statistically significant. (2) Adverse reactions: there were no adverse reactions in 3 reports included in the literature, and the other reports were not reported. Conclusion: FDP can significantly improve the effective rate and total effective rate of neonatal asphyxia after myocardial injury.
