不同复方短效口服避孕药治疗痛经的临床疗效分析

Effect of Different Conbination Oral Contraceptives in the Treatment of Dysmenorrhea

摘要: 目的:探讨4种短效避孕药治疗痛经的临床效果。方法:将我院231名患者随机分配到妈富隆组(A组)、达英-35组(B组)、优思明组(C组)、优思悦组(D组)及安慰剂组(F组)。应用TDS法、VAS法统计并观察各组在服药前、服用1周期、3周期、6周期时疼痛评分、疼痛缓解情况和不良情况发生率。结果:5组总有效率分别为90.2%、93.8%、95.9%、92.5%、16.7%,差异有统计学意义。痛经患者服用避孕药后1周期、3周期、6周期时TDS、VAS疼痛评分均较应用前明显降低,差异有统计学意义(P<0.05)。治疗后各组患者出现的总体不良反应率分别为45.1%、45.8%、42.9%、24.55%、6.7%,总体差异有统计学意义(χ2=19.216,P<0.05),卡方分割后发现优思悦组总不良反应率低于其妈富隆组、达英-35组和优思明组(χ2=4.859,P=0.028;χ2=13.296,P=0;χ2=3.849,P=0.005)。结论:短效口服避孕药治疗痛经疗效确切,而优思悦不良反应发生率相对少,治疗痛经的总体疗效也优于其余3种避孕药。

Abstract: Objective:To analyze clinical effect of the four kinds of short-acting contraceptives in the treatment of dysmenorrhea. Methods:231 patients with dysmenorrheal treated in our hospital from January 2017 to December 2018 were selected and randomly divided into group A,group B,group C,group D and group F according to the random number table method.group A,groupB,groupc,groupd and group F was given contraceptives Marvelon,Diane 35,Yasmin,YAZ and placebo for 6 cycles respectively.The effects of the five groups were observed and compared. Results:The total efficiency rate of the five groups was 90.2%、93.8%、95.9%、92.5% and 16.7% respectively,with statistically significant difference.The scores of TDS and VAS in group A,group B,group C and group D were significantly lower after 1, 3 and 6 cycles of contraceptive than before treatment.The overall adverse reaction rates were 45.1%, 45.8%, 42.9%, 24.55% and 6.7%, respectively. The overall difference was statistically significant (χ2=19.216, P<0.05).After chi-square segmentation, the total adverse reaction rate of the group D was lower than that of the group A, group B and group C (χ2=4.859, P=0.028; χ2=13.296, P=0; χ2=3.849, P=0.005). Conclusion:Short-acting oral contraceptives are efficient in treating patients with dysmenorrhea, while the incidence of adverse reactions ofYAZ is relatively lower. The overall efficacy of the treatment of dysmenorrhea is also better than the other three contraceptives.
