儿科医务人员输液认知及输液限制管理的影响因素研究

Study on the Influencing Factors of Infusion Cognition and Infusion Restriction Management among Pediatric Medical Staff

摘要: 目的:调查儿科医务人员输液知识储备情况,分析影响儿童门诊输液限制管理开展的因素,为提升儿童输液安全、减少不合理输液的发生提供建议。方法:采用多阶段分层整群抽样,以云南省为例对儿科医务人员进行输液知识及门诊输液限制管理影响因素的问卷调查。结果:共收到319份问卷,医务人员静脉输液知识平均得分为(13.70±3.09),不同年龄、职称、工作类别及月总收入医务人员间得分有统计学差异(P<0.05);在输液限制影响因素中,患者及家属因素得分最高,医院发展得分最低。结论:受访医务人员对输液知识了解程度总体较好,能清楚判断常见输液指征,但对感冒和炎症等指征的判断比较模糊,医疗机构还需进一步加强对医务人员的培训;同时应加强有效的医患沟通和宣传力度,结合相关疾病与政策因素,探索适宜的儿童门诊输液限制管理措施,减少不合理用药现象的发生。

Abstract: Objective:To investigate the transfusion knowledge reserve of pediatric medical staff and analyse the factors that affect the implementation of transfusion restriction management in children′s outpatient department, so as to provide reasonable suggestions for improving the safety of children′s transfusion and reducing the occurrence of unreasonable transfusion.Methods:A multi-stage stratified cluster sampling method was used to conduct a questionnaire survey on the infusion knowledge and outpatient infusion restriction management of pediatric medical staff in Yunnan Province.Results:A total of 319 questionnaires were received. The average score of intravenous infusion knowledge of medical staff was (13.70±3.09). There were statistical differences among medical staff of different ages, professional titles, job categories and total monthly income (P<0.05). Among the factors influencing infusion restriction, patients and family members scored the highest, while hospital development scored the lowest.Conclusion:The interviewed medical personnel generally have a good understanding of infusion knowledge and can clearly judge common infusion indications, but the judgment of cold, inflammation and other indicators is vague. Medical institutions need to further strengthen the training of medical personnel. At the same time, it is necessary to strengthen effective communication and publicity between doctors and patients, and explore appropriate management measures to limit transfusion in children′s outpatient service in combination with related diseases and policy factors, so as to reduce the occurrence of unreasonable transfusion.
