微生态制剂对结直肠癌术后化疗患者免疫功能和肠道菌群影响的Meta分析

Meta-analysis of Probiotics in Patients with Colorectal Cancer Undergoing Postoperative Chemotherapy

摘要: 目的:评价微生态制剂对结直肠癌(Colorectal Cancer,CRC)术后化疗患者免疫功能和肠道菌群平衡的影响,为微生态制剂在临床应用提供循证证据。方法:系统检索中、英文数据库,查找CRC患者使用微生态制剂联合化疗的随机对照试验(randomized controlled trial, RCT),应用Review Manager 5.3进行Meta分析。结果:纳入10篇国内RCT,共计966名患者,实验组483例,对照组483例。Meta结果表明:微生态制剂可以增加CRC化疗患者的CD4+数量(WMD:3.70,P<0.001)与CD4+ /CD8+的比值(WMD:0.39,P<0.001),降低CD8+含量(WMD:-3.84,P<0.001)、TNF-ɑ含量(WMD:-7.56,P<0.001)、IL-6含量(WMD:-9.40,P=0.04)、CRP含量(WMD:-21.79,P<0.001);可以升高双歧杆菌(WMD:1.38,P<0.001)、乳酸杆菌(WMD:1.18,P=0.02)和粪肠球菌(WMD:0.76,P<0.001)水平,而降低大肠杆菌(WMD:-0.70,P=0.001)水平。结论:对我国CRC患者,化疗期间使用微生态制剂可以减少患者化疗期间化疗药物所致的部分不良反应,预防肠道微生态系统失衡,增强患者的免疫力,改善患者化疗期间的生活质量。

Abstract: Objective:The study aimed to evaluate the immune function and intestinal flora balance of microecological probiotics in patients with Colorectal Cancer(CRC) undergoing postoperative chemotherapy and provide evidence-based evidence for the clinical application.Methods:A systematic search of the Chinese and English databases was conducted and randomized controlled trials for CRC patients using microecological agents combined with chemotherapy. Meta-analysis was performed using Review Manager 5.3 software.Results: A total of 966 patients were included in 10 domestic RCTS.There were 483 cases in both experimental group and control group.The Meta results showed that microecologics could increase the numberof CD4+(WMD: 3.70, P<0.001) and the ratio of CD4+/CD8+(WMD: 0.39, P<0.001), and reduce the content of CD8+(WMD: -3.84, P<0.001), TNF-Alpha content(WMD: -7.56, P<0.000 01), IL-6 content(WMD: -9.40, P=0.04), CRP content(WMD: -21.79, P <0.001); The levels of bifidobacterium(WMD: 1.38, P<0.001), Lactobacillus(WMD: 1.18, P=0.02) and Enterococcus faecalis(WMD: 0.76, P<0.001) were increased, while the levels of Escherichia coli(WMD:-0.70, P=0.001) were decreased. Conclusion: For Chinese CRC patients, the use of microecological preparations during chemotherapy can reduce part of the adverse reactions caused by chemotherapy drugs and prevent the imbalance of intestinal microecosystem, enhance the immunity of patients, and improve the quality of life of patients during chemotherapy.
