Evaluating the Economics of Peritoneal Dialysis as a Priority for Renal Replacement Therapy Based on Markov Model

摘要: 目的:评价腹膜透析作为肾脏替代治疗优先选择模式的经济学优势。方法:基于已发表的文献数据,构建马尔科夫模型,评价腹膜透析与血液透析相比作为肾脏替代治疗优先模式成本效用,通过与支付意愿阈值进行比较,判断腹膜透析作为肾脏替代治疗优选模式在我国治疗环境下的经济学。进行敏感性分析确定影响结果的关键影响因素。结果:在进行20年(贴现率5%)模拟后,血液透析相比腹膜透析增量成本为46 618元,增量效用为0.04 QALYs。血液透析相比腹膜透析的增量成本-效用比(ICUR)为1 165 450元/QALY,远大于3倍人均国内生产总值(GDP)217 341元。结论:根据目前的定价及我国经济水平,腹膜透析是具有成本-效用优势的肾脏替代治疗优选方案。

Abstract: Objective:To evaluate the economics of peritoneal dialysis(PD)as a preferred mode of renal replacement therapy(RRT). Methods:Based on published literature data, Markov model was constructed to evaluate the cost-utility of PD compared with hemodialysis(HD) as the preferred model of RRT, and the economics of PD as the preferred model of RRT in China was determined by comparing it with the threshold value of willingness to pay(WTP). Sensitivity analysis was performed to determine the key factors affecting the results. Results: After 20 years of simulation(discount rate 5%), the incremental cost of HD compared with PD was RMB 46618 and the incremental utility was 0.04 QALYs. Compared with PD, the incremental cost-utility ratio(ICUR) of HD was RMB 1,165,450 /QALY, which was much higher than three times of the gross domestic product(GDP) per capita. Conclusion: According to the current price and economics in China, PD is the best choice for RRT with cost-utility advantages.
