维生素D联合依托考昔对老年膝关节置换术后康复、骨代谢及炎症反应的影响

Effects of Vitamin D Combined with Etocoxib on Rehabilitation,Bone Metabolism and Inflammation in Elderly Patients after Knee Replacement

摘要: 目的:观察维生素D联合依托考昔对老年膝关节置换术(TKA)术后康复、骨代谢及炎症反应的影响。方法:采用随机数字表法将2020年6月至2022年6月我院行TKA老年患者98例分为对照组、联合组,各49例。对照组采用依托考昔治疗,联合组采用维生素D+依托考昔治疗。比较两组临床疗效、术后康复情况(膝关节功能、疼痛程度、膝关节活动度、康复质量)、骨愈合指标[骨保护素(OPG)、血管内皮生长因子(VEGF)、全甲状腺旁腺素(iPTH)]、骨代谢指标[总I型前胶原氨基端延长肽(tPINP)、骨钙素(BGP)、I型前胶原羧基端肽(PICP)、I型胶原羧基端肽β特殊序列(β-CTX)]、炎症因子[高敏C反应蛋白(hs-CRP)、白介素(IL-6、IL-2)、肿瘤坏死因子-α(TNF-α)]。结果:联合组临床总有效率(93.88%)高于对照组(79.59%)(P<0.05);与对照组相比,治疗后联合组VAS评分较低,膝关节活动度、SER评分、HSS功能评分较高(P<0.05);与治疗前相比,治疗后联合组VEGF、OPG明显升高,iPTH明显降低(P<0.05); 与治疗前比较,两组β-CTX、tPINP均降低,PICP、BGP均升高,且联合组更为显著(P<0.05);与治疗前相比,联合组治疗后hs-CRP、IL-6、TNF-α、IL-2降低幅度大于对照组(P<0.05)。结论:维生素D联合依托考昔治疗老年TKA术后患者疗效确切,可减轻炎症反应,缓解术后疼痛,还可改善骨代谢,加速骨愈合,促进膝关节功能恢复。

Abstract: Objective: To investigate the effects of vitamin D combined with etocoxib on rehabilitation, bone metabolism and inflammation in elderly patients after knee replacement (TKA). Methods: 98 elderly patients with TKA in our hospital from June 2020 to June 2022 were divided into control group and combination group, with 49 cases each. The control group was treated with etocoxime, and the combination group was treated with vitamin D+ etocoxime. The clinical efficacy, postoperative rehabilitation status [knee function, pain degree, knee mobility, rehabilitation quality], bone healing indexes [osteopagin (OPG), vascular endothelial growth factor (VEGF), total parathyroid hormone (iPTH)], bone metabolism indexes [total type I procollagen amino terminal prolonging peptide (tPINP), osteocalcin (BGP)] were compared between the two groups. Type Ⅰ procollagen carboxy-terminal peptide (PICP), type I collagen carboxy-terminal peptide β special sequence (β-CTX)], inflammatory factors [highly sensitive C-reactive protein (hs-CRP), interleukin (IL-6, IL-2), tumor necrosis factor-α (TNF-α)]. Results: The total effective rate of combined group (93.88%) was higher than that of control group (79.59%) (P<0.05). Compared with control group, VAS score of combined group after treatment was lower, knee motion, SER score and HSS function score were higher (P<0.05). Compared with before treatment, VEGF and OPG were significantly increased and iPTH was significantly decreased in combination group after treatment (P<0.05). Compared with before treatment, β-CTX and tPINP were decreased in 2 groups, PICP and BGP were increased, and the combination group was more significant (P<0.05). Compared with before treatment, hs-CRP, IL-6, TNF-α and IL-2 in combination group after treatment were decreased more than those in control group (P<0.05). Conclusion: Vitamin D combined with etocoxib is effective in the treatment of elderly patients after TKA, which can reduce inflammation, relieve postoperative pain, improve bone metabolism, accelerate bone healing, and promote the recovery of knee joint function.
