原研与仿制伏立康唑注射剂的 血药浓度对比与疗效分析

Blood Concentration Comparison and Therapeutic Effect Analysis of Original and Imitation Voriconazole Injection

摘要: 目的:探讨伏立康唑原研和仿制注射剂的血药浓度差异对住院患者治疗结局的影响,为临床个体化用药提供依据。方法:通过瑞美实验室管理系统(LIS),查询收集某院使用伏立康唑原研药和仿制药的住院患者病例,应用Excel 软件和SPSS 23.0软件对数据进行统计和分析。结果:伏立康唑原研药血药浓度在 >5.0 μg·mL-1所占比例最高(55.81%),仿制药血药浓度在(0.5~5.0)μg·mL-1所占比例最高(72.53%)。原研药和仿制药均是好转率最高,且二者的治愈情况、一般情况和危重情况基本一致; 仿制药好转率(53.85%)高于原研药(44.19%),原研药死亡率(9.30%)高于仿制药(1.10%)。二者在3个血药浓度范围的治愈率基本一致; 仿制药在(0.5~5.0)μg·mL-1范围的治愈率、好转率和一般率均优于其他两个浓度,且好转率、一般率均高于原研药; 原研药在>5.0 μg·mL-1范围的好转率和一般率均优于其他两个浓度。结论:仿制药在某院的临床使用率高于原研药,二者的疗效基本一致。伏立康唑仿制药的血药浓度更贴近有效血药浓度,原研药更易达到中毒浓度。

Abstract: Objective: To investigate the effect of blood concentration difference between Voriconazole and generic injection on therapeutic effect, and to provide basis for clinical individualized drug use. Methods: The cases of patients using voriconazole and generic drugs in a hospital were collected through Ruimei Laboratory management system(LIS), and the steady-state plasma drug concentration was measured by enzyme amplification immunoassay. The data were analyzed statistically by Excel and SPSS 23.0 software. Results: The proportion of Voriconazole original drug with blood concentration >5.0 μg·mL-1 was the highest(55.81%), and the proportion of generic drug with blood concentration of(0.5~5.0) μg·mL-1was the highest(72.53%). Both the original drug and the generic drug had the highest rate of improvement, and the cure, general condition and critical condition of both were basically the same. The recovery rate of generic drugs(53.85%) was higher than that of original drugs(44.19%), and the mortality rate of original drugs(9.30%) was higher than that of generic drugs(1.10%). The cure rates of the two drugs were basically the same in the three blood concentration ranges. The cure rate, recovery rate and general rate of the generic drug in the range of(0.5~5.0) μg·mL-1 were better than the other two concentrations, and the recovery rate and general rate were higher than the original drug. The improvement rate and general rate of the original drug in the range of >5.0 μg·mL-1 were better than the other two concentrations. Conclusion: The clinical utilization rate of generic drugs in a hospital is higher than that of original drugs, and the curative effect of the two drugs is basically the same. The blood concentration of Voriconazole generic drug is closer to the effective blood concentration, and the original drug is more likely to reach the toxic concentration.
