血必净注射液联合比阿培南对脓毒症休克患者血流动力学及微循环的影响

Effect of Xuebijing Injection Combined with Biapenem on Hemodynamics and Microcirculation in Patients with Sepsis Shock

摘要: 目的:探讨血必净注射液联合比阿培南在脓毒症休克患者中的应用效果。方法:选取2022年6月至2024年5月我院收治的83例患者脓毒症休克的临床资料分析,按照治疗方法不同分为常规组(41例)和观察组(42例),所有患者均给予常规支持疗法,同时常规组采用比阿培南治疗,观察组采用血必净注射液+比阿培南治疗。比较两组临床疗效、序贯器官衰竭评估(SOFA)评分、血常规[中性粒细胞(N)、C反应蛋白(CRP)、白细胞计数(WBC)、红细胞沉降率(ESR)]、血流动力学[平均动脉压(MAP)、心率(HR)、中心静脉压(CVP)]、微循环状态[中心静脉血氧饱和度(SCVO2)、中心静脉-动脉血二氧化碳分压差(Pcv-aCO2)、氧合指数、血乳酸]及预后指标。结果:观察组治疗总有效率90.48%较常规组73.17%升高(P<0.05);治疗3 d后,观察组SOFA评分较常规组降低(P<0.05);治疗3 d后,观察组N、CRP、WBC、ESR较常规组降低(P<0.05);治疗3 d后,观察组HR较常规组降低,MAP、CVP较常规组升高(P<0.05);治疗3 d后,观察组Pcv-aCO2、血乳酸较常规组降低,SCVO2、氧合指数较常规组升高(P<0.05);观察组ICU入住时间及28 d生存率与常规组比较无显著差异(P>0.05)。结论:血必净注射液联合比阿培南治疗脓毒症休克疗效显著,可改善患者血流动力学状态和微循环灌注,有效抑制炎症反应,改善患者健康状态。

Abstract: Objective:To explore the effect of Xuebijing injection combined with biapenem in the treatment of septic shock patients. Methods: The clinical data of 83 patients with sepsis shock admitted to our hospital from June 2022 to May 2024 were selected for analysis. According to different treatment methods, the patients were divided into a conventional group(41 cases) and an observation group(42 cases). All patients were given conventional supportive therapy, while the conventional group was treated with biapenem, and the observation group was treated with Xuebijing injection+biapenem. The clinical efficacy, Sequential Organ Failure Assessment(SOFA) score, blood routine examination [neutrophil(N), C-reactive protein(CRP), white blood cell count(WBC), erythrocyte sedimentation rate(ESR)], hemodynamics [mean arterial pressure(MAP), heart rate(HR), central venous pressure(CVP)], microcirculation status [central venous oxygen saturation(SCVO2), central venous-arterial partial pressure of carbon dioxide difference(Pcv-aCO2), oxygenation index, blood lactate] and prognostic indicators were compared between the two groups. Results: The total effective rate of the observation group was 90.48%, which was higher than that of the conventional group(73.17%)(P<0.05). After 3 d of treatment, the SOFA score in the observation group was decreased compared with the conventional group(P<0.05). After 3 d of treatment, the levels of N, CRP, WBC, and ESR in the observation group were decreased compared to the control group(P<0.05). After 3 d of treatment, the HR in the observation group was decreased compared with the conventional group, while MAP and CVP were increased compared with the conventional group(P<0.05). After 3 d of treatment, the Pcv-aCO2 and blood lactate levels in the observation group were decreased compared with the conventional group, while the SCVO2 and oxygenation index were increased compared with the conventional group(P<0.05). There was no significant difference in ICU admission time and 28 day survival rate between the observation group and the control group(P>0.05). Conclusion: The combination of Xuebijing injection and biapenem has significant therapeutic effects on sepsis shock, improving the patient′s hemodynamic status and microcirculatory perfusion, effectively inhibiting inflammatory reactions, and improving the patient′s health status.
