葡萄糖酸氯己定抗菌透明敷料与普通透明敷料在ICU中心静脉置管患者中应用的成本效果分析

Cost-Effectiveness Analysisof Tegaderm Chlorhexidine Gluconate(CHG) Dressing Versus Standard Dressing in Patients with Central Venous Catheter in Chinese Intensive Care Units

摘要: 目的:比较葡萄糖酸氯己定抗菌透明敷料(T-CHG)相对于普通透明敷料在我国ICU患者中心静脉置管护理中使用的经济性。方法:建立静态决策树模型,对1 000名患者进行队列模拟,模型终点考虑了相关并发症,包括导管相关性血流感染(CRBSI)、局部感染和皮炎。基础分析包括成本分析和成本-效果分析,并计算增量成本效果比。并进行单因素敏感性分析、情境分析以及概率敏感性分析以测试结果的稳定性。结果:基础分析结果表明,与普通透明敷料组相比,在单个患者水平,T-CHG敷料组和普通透明敷料组的总成本分别1 103.39元和1 323.69元,T-CHG敷料组可节约220.30元,具有成本优势。相应地T-CHG敷料组和普通透明敷料组的CRBSI发生率分别为1.51%和2.06%,T-CHG组可以降低0.55%的CRBSI发生率。单因素敏感性分析和情境分析结果与基础分析结果相同。概率敏感性分析结果表明,当支付意愿阈值从0增加到1万元时,T-CHG敷料组相对于普通透明敷料组存在61.1%~61.8%的概率具有经济性。结论:与普通透明敷料组相比,T-CHG抗菌敷料组总体成本更低,且可降低CRBSI发生率。

Abstract: Objectives: To estimate the economic impact of T-CHG dressing compared with standard dressing in patients withcentral venous catheters (CVC) in Chinese intensive care unit (ICU). Methods: A decision tree model was developed and a cohort of 1000 patients was simulated, in this model, we considered associated dermatitis, local infection at the catheter insertion site and CRBSI. Resource use, unit costs. Base-case analysis includes cost analysis and cost-effectiveness analysis, and calculation of incremental cost-effectiveness ratio (ICER).One-way sensitive analysis, scenario analysis and probabilistic sensitive analysis were conducted to test the robustness of the results. Results: In base-case analysis, the total costs of T-CHG dressing and standard dressing were 1 103.39 yuan and 1 323.69 yuan, respectively. Compared with standard dressing, T-CHGsaved 220.30 RMB/patient in patients with CVC in ICU. Correspondingly, the incidence of CRBSI was 1.51% and 2.06%, T-CHG could reduce 0.55% CRBSI rate, so T-CHG was dominant strategy.The results of the one-way sensitivity analysis and scenario analysis were consistent with base-caseanalysis. The cost-effectiveness acceptable curve showed that T-CHG had a 61.1% -61.8% probability of being cost-effectiveness choice compared to standard dressings when the willingness-to-pay threshold increased from 0 to 10,000 RMB. Conclusion: Compared with standard dressing, T-CHG havelower CRBSI rate and lower costsin patients with CVC in Chinese ICU.
