4种医学期刊2014—2019年发表的干预性IPD Meta分析方法研究

A Review of Method Approaches of Interventional IPD Meta-analysis Published in Four Major Medical Journals During 2014—2019

摘要: 目的:分析4种医学期刊2014—2019年发表的干预性单个病例数据(Individual Patient Data, IPD)Meta分析的特点及方法。方法:在PubMed上检索《新英格兰医学杂志》《柳叶刀》《美国医学会杂志》《英国医学期刊》2014—2019年间发表的干预性IPD Meta分析,并分析其方法学特点。结果:共检索到52篇文献,最终纳入28篇。纳入文献的研究领域主要集中在心血管及脑血管领域;纳入文献的IPD获得率超过80%的为16篇(57.1%),平均样本量达17 202人。共有18篇(64.3%)研究采用一步法综合IPD,18篇(64.3%)研究进行了生存时间数据分析,28篇(100%)研究均进行影响治疗效果的因素分析,17篇(60.7%)文献进行了敏感性分析。结论:IPD Meta分析可更有效研究临床异质性,其统计方法对数据处理也更加灵活,也可以更有效避免文献的偏倚,并更有效指导临床实践。但IPD Meta分析统计模型的多样性,在一定程度上反映了其统计方法的复杂性。本文通过对4种期刊发表的高质量IPD Meta分析方法的研究,可对未来开展IPD Meta分析提供有益的参考。

Abstract: Objective: To analyze the characteristics and methods of interventional Individual Patient Data (IPD) Meta-analysis published in four major medical journals during 2014-2019. Methods: We searched Pubmed for the interventional IPD Meta-analysis published in the New England journal of medicine, the Lancet, the Journal of the American Medical Association and the British Medical Journal between 2014 and 2019, and analysis the methodological characteristics. Results: 52 literatures were retrieved and 28 were eventually included. The published literatures mainly focused on cardiovascular and cerebrovascular. 57.1% of the IPD literature included in the study had a literature access rate of more than 80%. These studies with an average sample size of 17 202. Eighteen studies (64.3%) adopted the one-stage method to synthesize IPD, and survival data were used in 18 studies (64.3%) as the outcome indicators, 17 studies(60.7%) carried out sensitivity analysis. Conclusion: IPD Meta-analysis published in four major medical journals have large the sample size. The method can more effectively investigate clinical heterogeneity, and its statistical method is more flexible for data processing, which can also more effectively avoid literature bias. However, the diversity of statistical models of IPD Meta-analysis reflects the complexity of its statistical methods. Through the research on the high-quality IPD Meta-analysis methods, this paper can provide useful references for the future development of IPD Meta-analysis.
